Why aren't Endocrinologists in rural areas

There are so very many with autoimmune endocrine system diseases of persons of every age group, most of us have to travel long distances to find them.

WHY does every state have all the Endocrinologists located in one or two large clinics and hospitals?

I realize location is about funding, but if large clinics could rotate their doctors to rural clinics they would create an overload of patients that are now slowly dying because we are stuck seeing doctors that know nothing about these diseases.
